American Amicable Business Processing Instructions

• Applications are available on the American Amicable website
• You must be appointed in order to access the website (login and password required). For appointments please use our Appointment Application Request Form (All Carriers)
• New Business Fax Transmittal cover page - Must be submitted with application
• Fax only to 254-297-2100.
• Always fax originals only.
• Do NOT write in the margins, as this information may not be received during the fax transmission.
• ONLY fax one (1) application at a time; each application must have its own cover memo.
• To fax multiple applications: fax first application; hang up; redial to fax next application.
• Applications to be faxed in the following order: Fax Transmittal cover page, front and back of application (for two-sided applications), HIPAA form, Payment (echeck, void check, deposit slip, check), and any other supporting documents.
• Before faxing smaller items, such as a void check, make a copy on a full page, making sure it's placed at the top of the page.
• When feeding documents, make sure the tops of all documents are fed into fax machine first and all documents are facing in the same direction.
• Do NOT forward original application to Home Office unless instructed to do so by home office personnel.
• Keep the original application until the application has been approved and the policy delivered.
• Make sure to use the application with the correct state variations.

Methods of Payment

1. Collect Check with Application or use eCheck - The draft or check will be sent to the bank the next day after the application is received and then subsequent drafts will be on the requested draft day listed on the application. Echeck Bank Draft Authorization Form. No Cash On Delivery. Mail initial premium (client’s personal check) to: American Amicable P. O. Box 2549, Waco, TX 76702-2549 (reference case number on check)
2. Draft First Premium - This will not pay premium until the policy is issued by underwriting and until the requested draft day noted on the application. If no draft date is specified on the application, then the first draft will be drawn when the policy is issued.

Please Note: The first full modal premium is required with the application, unless the initial premium is bank draft. The initial premium can be submitted in the form of applicant’s personal check, eCheck, or bank draft for 1^st premium.
