Disability Insurance Processing Instructions
Disability applications must be Originals and must be mailed to the HBW home office along with Income Documentation, Paramed/Medical and the Illustration.

Submitting Disability Business "4 Easy Steps"
- Client Application (See Application Submission Rules below ) must be original signatures along with:
- Medical Requirements/Parameds are required for submitting all Disability policies. Please submit the required medical information along with the client app. Need to schedule a Paramed? Click here
- Income Documentation is required for submitting all Disability policies. The documentation required depends on your client's income source. (Non-owner, Owner of Sole Proprietorship, S Corp. Shareholder, Regular C Corp. Shareholder, Partnership, LLC or LLP.) DI carriers reserve the right to require additional financial information on any application regardless of amount if necessary to reach an underwriting decision.
- An Illustration is required to submit all Disability policies.

Application Submission Rules
- Originals are required and must be mailed to the HBW home office
- Use black ink. NEVER USE WHITE OUT
- Corrections must be crossed out and initialed by the client
- Make sure ALL the dates are the same (Including Premium check)
- Signatures and Dates Expire in 30 calendar days. Mail originals promptly!
- Answer all the questions pertaining to your client and their coverage
- Make sure the financials match the income documentation provided
- Income documentation must be submitted with the application
- Make sure the Illustration matches the paperwork (Elimination Period, Monthly
Benefit including riders, Benefit Period…)
- Illustration must be submitted with the application
- Medical/Parameds are required for most disability policies. Disability Carriers will
accept Life Insurance Exams, up to 6 months, with a cover letter. Additional
medical may be requested by the carriers to complete the underwriting process.
Disability business can be submitted without the medical but must be ordered
and submitted in a timely manner.
