• Fax only New Business Applications to OMFN New Business Department at (888) 858-4795.
• For amendments and other info pertaining to New Business fax to (402) 328-4021
• Life Application Fax Transmittal Form
• Client Application (Follow the first page instructions)
• Band Draft Form (EFT) and Voided Check (If paying monthly)
• Paramedical Exam and Lab Ticket must be ordered by the agent (copy of medical, Part II not required to be faxed with application)
• HIPPA Form (Not required in all states. If you're not sure send it)
• HIV Consent Form
• Replacement form (If you are replacing a policy)
• Any additional state specific and/or supplemental forms that are required.

Remember - You must retain copies of all documents for your files!
