Disability Division Processing Instructions

Applications must be originals (NO COPIES)

Submitting Disability Business "4 Easy Steps"

  1. ORIGINAL client application (See Application Submission tips and tricks below)
  2. Income Documentation will be required for all Disability policies. The documentation required depends on your client's income source. (Non-owner, Owner of Sole Proprietorship, S Corp. Shareholder, Regular C Corp. Shareholder, Partnership, LLC or LLP). DI carriers reserve the right to require additional financial information on any application regardless of amount if necessary to reach an underwriting decision.
  3. An Illustration is required to submit all Disability policies.
  4. Medical Requirements/Parameds can be ordered after submitting the application, income docs and illustration. In some cases medical will not be required.

Application Submission Tips and Tricks

  • Originals are required and must be mailed to the DIS home office
  • Signatures and Dates Expire in 30 calendar days. Mail originals promptly!
  • Use black ink. NEVER USE WHITE OUT
  • Corrections must be crossed out and initialed by the client
  • Make sure ALL the dates are the same (including premium check)
  • Answer all the questions pertaining to your client and specific coverage
  • Income documentation must be submitted with the application
  • Make sure the financials match the income documentation provided
  • Illustration must be submitted with the application
  • Make sure the Illustration matches the paperwork (Elimination Period, Monthly Benefit including riders, Benefit Period)
  • Medical/Parameds are required for most disability policies. The medical can be order after the application is submitted. Disability Carriers will accept Life Insurance Exams, up to 6 months, with a cover letter. Additional medical may be requested by the carriers to complete the underwriting process. Disability business can be submitted without the medical but must be ordered and submitted in a timely manner
