If pre-appointment is required, the client application should be submitted AFTER you are appointed/contracted with the carrier.
For Life Brokerage appointments, log in and download the appropriate form on the forms page.

Complete the WCL EZapp and fax to HBW. View Checklist
WCL will call your client for a phone interview within 24hrs.
WCL will order the medical if necessary.

Complete part 1 of the application.
Complete the Agent's Report.
If temporary insurance is desired please follow the instructions on the front of the application "Do and Do Not"
Complete EFT Form if monthly draft is desired.
Complete HIV Form.
Complete HIPPA Form.
Complete Supplement to Life App Part 1.
Package medical/paramed with application.
